Perceived as a simple act, voluntary termination of pregnancy by medication is accompanied by pain of varying intensity that can be avoided by analgesics.

Since its authorization in 1990, the medical technique has been increasingly used in the context of voluntary termination of pregnancy (abortion) (16% of the 209,000 abortions performed in France in 1992, but 57% of the 220,000 abortions in 2015). To find out more about these poorly studied pains, Insert and the Clotilde Vattier abortion center in Nantes conducted a survey of 453 women. "This study is a call to take better account of pain during this procedure, which concerns more than 130,000 women in France, like any medical practice," emphasizes Dry Philippe David, gynecologist at the Clotilde Vattier abortion center and initiator of this research project, conducted by the Foundation de l'Avenir.

The results, published this month, show that 27% of women who had a medical abortion experienced very severe pain on the third day of treatment (equal to or greater than 8 on a scale of 0 to 10). "We noticed that the women who felt the most pain were those who had never had a pregnancy, those who were used to having painful periods, or those who received a 200 mg dose of Mifepristone, rather than a 600 mg dose as is done in some centers," explains Dr. Philippe David.

In addition to severe bleeding and pain, 94% of the 453 women surveyed reported at least one treatment side effect within five days of the abortion. The most common was fatigue (88%), followed by nausea (70%), dizziness (42%), and diarrhea (37%) and vomiting (28%). To cope with these pains, 83% of the women in the study reported taking pain medication during the five days of treatment.

The study also found that taking pain medication shortly before treatment helps reduce pain. In addition, one-third of the women surveyed said they were concerned about bleeding that accompanies the expulsion of the egg. Some also mentioned the difficulty of carrying out normal activity during this period and recommended several days of rest.
